1.) The Town Clerk’s Office possess Death Records of individuals who died in the Town or Village of Catskill
2.) The Applicant must be an immediate family member of the deceased (i.e. Spouse, Parent, or Child)
a.) Other individuals who can obtain death records are those with a New York State Court Order or documented lawful right or claim.
3.) Applicant must complete the appropriate form at the Town of Catskill Clerk’s Office
4.) Proof of Relationship to the deceased is required
5.) Photo Identification is required
